Terms and Conditions for Consultant Psychiatry Services

1. Introduction
This document outlines the terms and conditions for engaging with Dr John Roche, a Consultant Psychiatrist ("the Psychiatrist"). By scheduling an appointment, you ("the Patient") agree to abide by the terms and conditions outlined in this agreement.
2. Scope of Service
The Psychiatrist provides outpatient psychiatric services that include but are not limited to assessments, diagnoses, and treatment plans for ADHD, eating disorders, and general psychiatry.
3. Limitations of Service
a. Suicide Risk and Emergency Treatment: The Psychiatrist is not equipped to handle cases with a high risk of suicide or to provide emergency psychiatric treatment. If you are experiencing a psychiatric emergency, it is imperative to seek emergency medical treatment immediately.
b. Patient Acceptance: An initial assessment does not necessarily mean that the Psychiatrist will be able to accept you as an ongoing patient.
4. Appointments
a. Scheduling: Appointments must be scheduled in advance.
b. Cancellation Policy: Please provide at least 24 hours’ notice for cancellations. Failure to do so will result in a cancellation fee of 50%.
5. Fees and Payments
a. Payment is due at the time of booking the appointment
b. Various payment methods are accepted, details of which will be provided upon scheduling an appointment.
6. Patient Discharge
If there is a lapse of more than 3 months between appointments without prior arrangement, the Patient will be considered discharged from the Psychiatrist’s services.
7. Confidentiality
All patient information will be held in the strictest confidence in compliance with data protection laws and professional ethical guidelines.
8. Governing Law
This agreement is governed by the laws of the United Kingdom.
9. Amendments
The Psychiatrist reserves the right to amend these terms and conditions at any time. Any amendments will be effective upon being posted to the Psychiatrist’s official website or otherwise communicated to the Patient.
10. Acknowledgment
By scheduling an appointment, you acknowledge that you have read, understood, and agree to be bound by these terms and conditions.
